Inhouse Hospice Llc in Michigan is seeking an experienced Registered Nurse Case Manager to join our hospice team. This role manages a patient caseload of 12-15, coordinates interdisciplinary care, and ensures comfort and dignity for patients and end‑of‑life.

Hybrid remote in Bingham Farms, MI with a 75% travel expectation. The ideal candidate holds an active Michigan RN license with hospice/home health experience and strong communication and compassion for end-of-life care.

Qualifications

• Active RN license in Michigan (required).
• Minimum 1 year hospice or home health experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and compassion for end-of-life care.
• Reliable transportation, valid driver's license and car insurance.

Responsibilities

• Manage and coordinate care for hospice patients in Oakland County.
• Perform comprehensive nursing assessments and develop individualized care plans.
• Collaborate with physician, social workers, aides and chaplains.
• Provide education and emotional support to patients and families.
• Ensure compliance with Medicare hospice regulations and documentation standards.
• Participate in education groups at facilities.
