Job summary

VNA Health in Santa Barbara, CA is seeking an experienced Registered Nurse Case Manager (RN CM) to plan, organize, and direct hospice services within patients’ homes and communities. The role emphasizes community health education and collaboration with the hospice interdisciplinary team to ensure high-quality end-of-life care.

Qualifications

• Current and active California RN license.
• Bachelor’s degree, preferred.
• Minimum one year acute care experience required.
• One year recent hospice experience preferred.
• Current CPR certification.
• Licensed driver with insured automobile - in accordance with state and/or organization requirements.
• Management experience not required.
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
• Ability to respond independently to life threatening situations.

Responsibilities

• Provide comprehensive case management to hospice patients.
• Complete initial assessment of patient and family to determine hospice care needs and admits to appropriate level of service.
• Assess and evaluate patient status; write and initiate plan of care and regularly re-evaluate needs.
• Develop a care pathway with goals and nursing actions, include patient and family.
• Initiate preventive and rehabilitative nursing procedures and administer prescribed medications.
• Counsel patient and family in meeting nursing needs and provide health instructions.
• Assist with activities of daily living and promote self-sufficiency and comfort care.
• Ensure compliance with state/federal/local regulations for hospice services.
