The position offers a full-time schedule with an on-call rotation, a competitive salary in the range of $80,000—$85,000, and various benefits including medical insurance and paid time off.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State License as a Registered Nurse.
• Minimum one (1) year experience as a nurse in a clinical care setting.

About the Role

As a Hospice RN Case Manager you will provide skilled nursing care and compassionate support to patients and families during life's final chapter. You will build relationships with patients, families, physicians, facilities and interdisciplinary teams, coordinating comprehensive care while addressing physical, emotional and spiritual needs.

This role is a high‑travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities or nursing homes.

Hours

• Full Time: Monday to Friday, 8 a.m. to 5 p.m., with a weekend on‑call rotation
• Part Time: Three weekdays, 8 a.m. to 5 p.m., with a weekend on‑call rotation
• Per Diem: Flexible weekdays, Monday to Friday, 8 a.m. to 5 p.m., with a weekend on‑call rotation

Required Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State License as a Registered Nurse
• Minimum one (1) year experience as a nurse in a clinical care setting
• Reliable transportation, valid driver's license and automobile insurance coverage

Preferred Qualifications

• Bachelor of Science in Nursing (BSN)
• Hospice or home health experience

Pay Range

$80,000—$85,000 USD
