Job summary

AccentCare in Portland, OR seeks a licensed RN to join the team as a Case Manager. You will coordinate patient care, manage caseloads, and participate in on-call rotations to ensure continuous, high-quality support.

The role requires an active RN license, CPR certification, and experience in hospice or home health environments. Candidates should be prepared to meet state regulatory requirements and uphold AccentCare's TRUE HOPE values.

Qualifications

• RN license is required in the state(s) where the program operates.
• Current and valid CPR certification; required in Georgia.
• Minimum one year of nursing experience; required in California.
• Experience in hospice, home health, or geriatrics nursing.
• Willingness to serve on-call.
• Must have reliable automobile, valid driver's license, and required auto insurance.
• Meet the regulations and requirements of the State in which the program provides services.

Responsibilities
