Job summary

AccentCare is seeking a dedicated Case Manager in Loveland, CO to provide case management for hospice and home health patients. The role requires RN licensure in the practicing state, willingness to be on-call, and a reliable vehicle to service patients in the area.

The position offers a $10,000 sign-on bonus and a pay range of $56,200–$89,900 per year. CPR certification is preferred and strong commitment to the AccentCare mission will be valued.

Qualifications

• Associates degree preferred; RN license required in practicing state(s).
• Current CPR certification preferred—required in Georgia.
• Minimum one year of nursing experience, preferred—required in California.
• Previous experience in hospice, home health, general medical-surgical care, or patient-population-specific nursing.

Responsibilities

• Provide case management for hospice and home health patients.
• Be on-call as needed to coordinate patient care.
• Support Pre-Hospice patients and transition planning.
