Job summary

St. Mary’s Home Care and Hospice is seeking a full-time Hospice RN Case Manager in Georgia. You will manage a caseload of hospice patients, collaborate with an interdisciplinary team, and participate in weekly meetings to coordinate patient care.

The role includes interaction with physicians and the hospice medical director as needed to control symptoms. The position is Monday–Friday with no on-call or weekend duties. A $10K signing bonus is offered, with some internal exceptions noted.

Qualifications

• Graduate of an approved nursing education program
• Current GA RN license
• At least 2 years of nursing experience in acute care, home care, or ER
• EPIC experience preferred

Hospice RN Case Manager position summary

Responsible for case managing a team of hospice patients. RN case manager will collaborate with interdisciplinary team and attend weekly Interdisciplinary meetings. RN Case manager will interact with the primary care physician and/or hospice medical director as needed to effectively manage patient symptoms. Position is full time primarily Monday-Friday. Minimum qualifications

• Graduate of an approved nursing education program

• Licensure as a Registered Nurse in the State of Georgia

• 2 years experience as a professional nurse in acute care, home care or ER experience required

• Home care experience preferred but not required

• EPIC experience preferred
