Job summary

Shalom Hospice of Washington is seeking a compassionate Registered Nurse (RN) Case Manager to join our team in and around Kitsap County. You will plan, organize, and direct hospice care for terminally ill patients and families, providing physical, emotional, and spiritual support.

The role involves comprehensive patient assessments, nursing care, and updating care plans as needs change, with on-call duties and potential supervision of ancillary staff.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with a license in good standing.
• One year as a professional nurse within the last three years or a BSN with two years' experience.
• Excellent observation, verbal and written communication skills, problem solving, basic math, and nursing skills.
• Ability to lift, position, and transfer patients and equipment.
• Possesses and maintains CPR certification.

Responsibilities

• Completes comprehensive assessments of patient and family to determine hospice needs.
• Provides professional nursing care utilizing the nursing process.
• Initiates and revises plan of care as patient status and needs change.
• Maintains accurate clinical notes and communicates with physicians and families.
• Reports abuse, neglect, or financial exploitation as required by law.
• Participates in on-call duties and supervises ancillary personnel.
