Job summary

Sanctuary Hospice is seeking a compassionate Registered Nurse Case Manager (RNCM) to join our per diem nursing team providing end-of-life care across Summit, Portage, Medina, Stark, and Wayne counties. You will coordinate services and support patients and families through every step of their hospice journey.

As an RN Case Manager, you will deliver hands-on nursing care, partner with our interdisciplinary team, and advocate for comfort, dignity, and quality of life.

Qualifications

• RN license required
• Minimum one year RN experience in medical-surgical/acute care
• Proficient with EMR and computer use
• CPR certification if state requires
• Valid driver's license and reliable transportation

Responsibilities

• Respond to clinical referral information with care and expertise.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Provide direct nursing care and coordinate services for patients.
• Deliver nursing care rooted in proven principles to relieve pain and discomfort.
• Educate patients and families to enable active participation in care plans.
• Participate in on-call duties as part of the team.
