Job summary

Agrace is seeking an RN Case Manager to deliver patient- and family-centered care by conducting assessments, planning and updating the care plan, and coordinating interventions. This role emphasizes collaboration with the interdisciplinary team to ensure quality hospice and palliative care.

Responsibilities include patient assessments, care planning, and supervising nursing staff while maintaining compliance with policies.

Qualifications

• ADN or BSN required.
• Current, unrestricted Registered Nursing License in state of employment.
• One-year nursing experience, preferred.
• Valid driver’s license, auto insurance, and access to a reliable vehicle.
• Must be at least 18 years of age.
• Basic Microsoft Office Skills.
• Intermediate electronic health records skills.
• Ability to use proper body mechanics, safe patient handling, and standard precautions.

Responsibilities

• Assess patient’s and family’s needs and document findings in the plan of care.
• Provide nursing interventions, pain management, and education to patients and families.
• Develop and revise the plan of care with the Interdisciplinary Team and family.
• Oversee CNA and LPN support and delegate visits as appropriate.
• Precept new staff and students.
