Agrace in Watertown, WI is seeking a Nurse Case Manager (RN) to deliver patient- and family-centered hospice care. You will assess needs, plan and direct care, and document progress in the EHR, coordinating with the interdisciplinary team.

This full-time day-shift role includes on-call rotations, mileage reimbursement, and comprehensive benefits. A BSN/ADN and an active RN license in Wisconsin are required, with at least one year of nursing experience and a valid driver's license.

Qualifications

• ADN or BSN required.
• Current, unrestricted RN license in Wisconsin.
• At least 1 year nursing experience preferred.
• Valid driver's license and access to a reliable vehicle.
• Must be at least 18 years old.
• Basic MS Office and intermediate EHR skills.

Responsibilities

• Assess patient and family needs per the plan of care.
• Provide interventions, pain management, and education.
• Develop and revise the plan of care with the IDT.
• Document interventions and outcomes in the EHR.
• Oversee CNAs and LPNs; delegate visits as needed.
• Participate in meetings to plan and evaluate services.
