Job summary

Trustbridge Hospice, part of Empath Health, seeks a compassionate Hospice RN Case Manager to join our Palm Beach County team. Monday–Friday 8:30am–5:00pm with weekend coverage; provide patient-centered care and guide families through life’s journey in a supportive, community-based setting.

You’ll collaborate with an interdisciplinary team, develop individualized care plans, monitor conditions, and educate families to enhance comfort and quality of life across Florida’s Palm Beach area.

Qualifications

• Must be a Florida-licensed Registered Nurse.
• Current Basic Life Support (BLS) certification.
• Hospice case management, crisis intervention, and family teaching.
• Proficient with computer/EMR systems.
• Reliable transportation and valid Florida driver's license.

Responsibilities

• Advocate for patients and families, ensuring needs are met.
• Coordinate care under the physician and IDG.
• Develop and implement individualized care plans.
• Monitor patient conditions and provide interventions.
• Educate families to improve comfort and quality of life.
