Qualifications

• Graduate of an accredited nursing program; Associate degree or higher preferred.
• Current RN license in the state of practice or valid multi-state license.
• CPR certification required; valid driver’s license.
• Hospice, palliative care, or home health experience preferred.

Responsibilities

• Complete initial and ongoing assessments of patients’ needs and environment.
• Coordinate with physicians for certification and recertification per Medicare.
• Serve as clinical point person for assigned patients with symptom management.
• Develop and update individualized care plans with patients, families, and IDG.
• Document interventions and outcomes accurately; ensure regulatory compliance.
• Educate patients and families on disease progression, medications, and hospice philosophy.
• Supervise hospice aides and revise care plans as needed.
• Participate in IDG meetings with clinicians and social work staff.
• Respond to urgent needs during on-call rotation.
