Qualifications

• Bachelor’s degree or higher in a health-related field.
• Minimum of 3 years of clinical RN experience, with hospice experience required.
• At least 1 year of supervisory or management experience in a hospice setting.
• Strong knowledge of Medicare Conditions of Participation, Local Coverage Determinations, and regulatory standards.

Responsibilities

• Leadership & Team Oversight – supervise and mentor the hospice care team.
• Patient & Family Coordination – lead interdisciplinary team members and coordinate care.
• Case Management – oversee care plans, evaluations, and treatments.
• Documentation & Compliance – review care plans in EMR for accuracy and Medicare adherence.
• Quality Improvement – participate in QA initiatives and drive performance improvements.
• Interdisciplinary Team Meetings – lead meetings with clinicians and medical director.
