Job summary

Anchor Health is seeking a dedicated RN Case Manager to coordinate hospice care plans for patients in the community. The role requires active nursing practice, care planning, and collaboration with families and the interdisciplinary team across home settings.

Qualified candidates hold a CA RN license and CPR certification, with Hospice experience preferred. Must have reliable transportation and the ability to drive to patient locations on a regular basis.

Responsibilities
