Job summary
Pathways Hospice is seeking a Registered Nurse Case Manager for a full-time role covering the Northwest, Southwest & Southeast Houston areas. You will lead the nursing care team, collaborate with physicians and therapists, plan home health services, and educate patients and their families to improve outcomes.
The position requires an TX RN license and hospice experience; HomeCare HomeBase EMR experience is a plus. Competitive benefits and on-call rotation are included.
Qualifications
- RN license in Texas is required; hospice experience preferred.
- Knowledge of home health regulations and compliance is important.
- Experience with HomeCare HomeBase (HCHB) EMR is preferred.
Responsibilities
- Oversee nursing care team and coordinate patient care.
- Communicate with physicians, therapists, and healthcare professionals to ensure cohesive care.
- Plan, organize, and direct home health services.
- Conduct patient assessments and develop care plans.
- Educate patients and families on health management and medications.
- Prepare timely clinical notes and maintain documentation.
- Night and weekend coverage required; on-call rotation weekly.
