Grace Hospice is seeking a dedicated RN Case Manager to provide end-of-life care in patients' homes or community settings. You will coordinate nursing and hospice aide services, ensuring quality, compassionate care and clear communication with families and the care team.

The role requires the ability to plan and adapt care, on-call availability, and adherence to the 4Ms framework to support patient-centered goals and comfort.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Valid driver’s license and proof of auto insurance.
• Two years of RN experience.
• Two years of Hospice/Palliative Nursing experience preferred.

Responsabilités

• Provide end-of-life care and hospice services in patients' homes or community settings.
• Supervise and instruct hospice aides in patient care.
• Maintain care coordination notes and visit documentation.
• Assist with medication delivery after hours, weekends, and holidays.
• Attend interdisciplinary team meetings during on-call periods.
• Admit patients eligible for hospice services under physician order.
• Develop patient care plans addressing identified problems and goals.
• Apply 4Ms framework: What Matters, Medications, Mentation, Mobility.
