Beacon Hospice, Amedisys Optum in Connecticut is seeking a Hospice Registered Nurse Case Manager to coordinate compassionate, patient-centered hospice care. As an RN, you will assess patients, partner with the interdisciplinary care team, and develop plans tailored to each individual's needs.

Responsibilities include performing patient assessments, collaborating on care plans, documenting progress, managing pain and symptoms, teaching patients and caregivers, and participating in on-call

Competenze

• Current RN license valid in the state of assignment.
• Current CPR certification.
• At least 1 year of nursing experience.
• Valid driver's license with reliable transportation and liability insurance.

Mansioni

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness, and according to care center policies.
• Records pain/symptom management changes and outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with the clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
