Ascend Hospice & Palliative Care MA is excited to hire an RN Case Manager to support on-call coverage, case management and patient visits in Framingham, MA. The RN will perform initial patient assessments, develop and adjust care plans, collaborate with the healthcare team, and educate patients and families on end-of-life needs.

Travel is required as needed and strong communication skills are essential. This role offers a starting salary around $93,600 with competitive benefits and opportunities

Formación

• Initial patient assessment to determine hospice needs
• Regularly re-evaluates patient nursing needs
• Develops and updates care plans
• Collaborates with healthcare team to coordinate care
• Counsels patients and families on end-of-life needs
• Initiates appropriate nursing procedures and care
• Communicates patient needs to physicians and reports changes
• Supervises Hospice Aides and supports team
