Ascend Hospice is seeking an RN Case Manager in Concord, MA for a great career opportunity in hospice. The starting salary is $93,600 with flexible and personalized scheduling.

This full-time position offers Monday through Friday hours with no overnights or weekends, optimizing work-life balance and patient-centered care in a supportive environment. You will assess patients and families, collaborate with the healthcare team, and supervise Hospice Aides to ensure continuous, well-coordinated

Qualifications

• Must hold State Certified RN license.
• 2 years' experience as a professional nurse, hospice experience preferred.
• Must have a valid driver's license and reliable transportation.
• Must be willing to travel.
• Strong verbal and written communication skills.

Responsibilities

• Complete an initial assessment of patient and family to determine hospice care needs.
• Regularly re-evaluates the patient’s nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the entire healthcare team, patient and family, to direct the work of the team to provide care that is continuous and well-coordinated.
• Counsels the patient and family in meeting end of life needs.
• Initiates appropriate preventative and palliative nursing procedures.
• Communicates with the physician regarding patient’s needs and reports any changes in the patient’s condition.
• Supervises the Hospice Aides.
