Job summary

Jobot is seeking a Hospice RN Case Manager in SOUTH Brooklyn, NY. The role involves four daily visits, home- and field-based care, and close coordination with an interdisciplinary team to deliver end-of-life care.

The position requires an active NY RN license with hospice or related experience; BSN is preferred and a car is mandatory for territory travel. Competitive benefits and a $15k sign-on bonus accompany the salary.

Qualifications

• RN must hold an active NY State RN license and have hospice/palliative care or Med Surg/Oncology experience.
• BSN degree preferred.
• RN must have access to a car.

Responsibilities

• Provide skilled nursing care to terminally ill patients in home and facility settings.
• Manage caseload with four hospice visits per day and coordinate care plans.
• Develop and follow Hospice Plan of Care focusing on comfort and quality of life.
• Educate patients and families on disease progression and symptom management.
• Collaborate with physicians and IDT to adjust medications and plans.
• Maintain accurate clinical documentation and inventory of equipment.
