Job summary

Jobot is seeking a Full-time Hospice Admissions RN to cover Queens and Brooklyn, NY. The role includes an initial health assessment, care planning, and hospital/home/nursing facility admissions following hospice standards.

The schedule is Monday through Friday, 11 AM to 7 PM. Qualifications include NY RN licensure (BSN preferred), 2+ years in Medical-Surgical or Community Health (Hospice/Oncology preferred), CPR-BLS, and a NYS driver's license. Employee benefits are extensive.

Qualifications

• Graduate from an accredited School of Nursing; BSN preferred.
• Minimum two years’ experience in Medical-Surgical or Community Health; Hospice/Oncology preferred.
• New York State RN licensure required.
• CPR-BLS and valid NYS Driver's license preferred.

Responsibilities

• Perform an initial physical, psychosocial, and health systems assessment in patients' homes, hospitals, or nursing facilities to develop a plan of care and goals.
• Admit patients following National Hospice Organization standards and ensure the admission process meets regulatory requirements.
• Engage in community outreach to physicians and medical providers to share Hospice information and support performance improvements.
• See 2–3 patients per day; follow up per shift and coordination with care team.
