Job summary

Jobot is seeking a Full-time Hospice RN Case Manager in Queens, NY. The RN handles a small territory (3 zones/ZIPs) and will see four hospice patients daily, delivering skilled nursing care in home and facility settings, coordinating hospice services per the plan of care.

Responsibilities include care planning, patient/family education, collaboration with physicians and the IDT, and comprehensive documentation. NY RN license required; BSN preferred; 15K sign-on and robust benefits included.

Qualifications

• RN with active NY State RN License in good standing and at least 1 year RN experience in Hospice or palliative care, Med Surg or Oncology (car)
• BSN degree highly preferred

Responsibilities

• See four Hospice patients per day and manage caseload of end-of-life patients in home and facility settings
• Create and follow plan of care to maximize comfort and quality of life
• Provide physical and psychosocial health assessments to develop Hospice Plan of Care and Goals of Care
• Follow infection control standards
• Educate patients and families on disease progression, symptom management and medication administration
• Collaborate with Physicians to titrate medications and update care plans with the IDT
• Assess equipment needs and coordinate with vendors for supplies and delivery
• Maintain accurate and timely clinical documentation
