Job summary

Serenity Hospice PA is seeking a compassionate and dedicated Hospice RN Case Manager (RNCM) to join our growing team serving patients throughout Dauphin County, PA.

This role provides end-of-life care at patients' homes, coordinating care plans, performing comprehensive nursing assessments, symptom management, and patient education, while collaborating with physicians and an interdisciplinary team. No nights or weekends; mileage reimbursement and benefits available.

Qualifications

• Active Pennsylvania RN license.
• Hospice or home health experience preferred.
• Strong clinical and organizational skills.
• Excellent communication and compassionate patient care abilities.
• Valid driver’s license and reliable transportation.

Responsibilities

• Manage and coordinate hospice patient care plans.
• Perform comprehensive nursing assessments.
• Provide symptom management and patient education.
• Collaborate with physicians, caregivers, and interdisciplinary team members.
• Maintain accurate and timely documentation.
• Support patients and families emotionally throughout the hospice journey.
