Serenity Hospice PA is seeking a compassionate Hospice RN Case Manager to join our Montgomery County team. You will coordinate care, manage symptoms, educate families, and work with an interdisciplinary team to support patients and their loved ones.

Qualifications

• Active Pennsylvania RN license is required.
• Strong clinical assessment and organizational skills are essential.
• Excellent communication and interpersonal abilities required.
• Compassionate, patient-centered approach expected.
• Ability to work independently and as part of a team.
• Valid driver’s license and reliable transportation preferred.

Responsibilities

• Manage and coordinate individualized hospice patient care plans.
• Perform comprehensive nursing assessments and ongoing patient evaluations.
• Provide expert symptom management and compassionate nursing care.
• Educate patients and families about disease progression, medications, and comfort-focused care.
• Collaborate with physicians, caregivers, and interdisciplinary team members.
• Maintain accurate, timely, and thorough clinical documentation.
• Provide emotional support and guidance to patients and loved ones.
• Advocate for patient comfort, dignity, and quality of life.
