Serenity Hospice PA is seeking a compassionate Hospice RN Case Manager (RNCM) to join our growing team serving patients throughout Dauphin County, PA. The role focuses on delivering end-of-life care in patients' homes, coordinating care plans, and supporting families with education and guidance.

The ideal candidate holds an active Pennsylvania RN license, hospice or home health experience is preferred, and possesses strong clinical and organizational skills.
