Serenity Hospice PA in Philadelphia, PA is seeking a compassionate Hospice RN Case Manager (RNCM) to coordinate end-of-life care for patients and families. You will be the trusted clinical presence, working closely with physicians, caregivers, and the interdisciplinary team to deliver individualized, compassionate care.

This role offers a supportive work environment with no nights or weekends, mileage reimbursement, PTO, health benefits, and 401(k) enrollment as part of a growing hospice

Qualifications

• Active Pennsylvania RN license required.
• Strong clinical assessment and organizational skills.
• Excellent communication and interpersonal abilities.
• Compassionate, patient-centered approach to care.
• Ability to work independently while collaborating with a team.
• Valid driver’s license and reliable transportation.

Responsibilities

• Manage and coordinate individualized hospice patient care plans.
• Perform comprehensive nursing assessments and ongoing patient evaluations.
• Provide expert symptom management and compassionate nursing care.
• Educate patients and families regarding disease progression, medications, and comfort-focused care.
• Collaborate with physicians, caregivers, and interdisciplinary team members.
• Maintain accurate, timely, and thorough clinical documentation.
• Provide emotional support and guidance to patients and their loved ones.
• Advocate for patient comfort, dignity, and quality of life.
