Ambercare Hospice is seeking a compassionate Registered Nurse Case Manager to join our team in Silver City, NM. You will provide expert clinical care and support to patients and families, ensuring comfort, dignity, and quality of life.

Qualifications

• Graduate from an accredited registered nursing program.
• RN license active in state of practice.
• Current CPR and First Aid certifications.
• Valid driver’s license and proof of insurance.
• Knowledge of quality assessment and assurance procedures.

Responsibilities

• Manage patient and family caseload throughout nursing processes: assessment, admission, planning, implementation, evaluation.
• Gather data from patient and family factors to inform care plans with the team.
• Communicate with attending physician and interdisciplinary team to coordinate care.
• Provide regular visits to reassess needs and update the plan of care.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records, charts, and care plans.
• Supervise and document Home Health Aide delivery per guidelines.
