Ambercare Hospice is seeking a compassionate and dedicated Registered Nurse Case Manager to join our team in Santa Fe, NM. You will provide expert clinical care and support to patients and families during meaningful moments, helping ensure comfort, dignity, and quality of life.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Current CPR and First Aid certifications.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation.
• Obtain data from patient physical, psychological, social, and spiritual factors and initiate interventions by the interdisciplinary team.
• Initiate communication with the attending physician and other agencies to coordinate optimal care.
• Provide regular visits to reassess the patient’s needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records, charts, progress notes, and timely care plans.
• Supervise and document delivery of care by the Home Health Aide as required by guidelines.
