Ambercare Hospice in Santa Fe, NM seeks a compassionate Registered Nurse Case Manager to provide expert clinical care to patients and families, ensuring comfort, dignity, and quality of life during meaningful moments. You will manage patient and family caseloads, assess needs, coordinate with the interdisciplinary team, and support families through the care journey.

Qualificações

• Graduate from an accredited registered nursing program.
• Licensed RN in the state of practice.
• Must possess current CPR and First Aid Certification.
• Must be knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance.

Responsabilidades

• Manage primary patient and family caseload across assessment, admission, planning, implementation, and evaluation phases of the nursing process.
• Obtain data and assess information from patient and family across physical, psychological, social, and spiritual factors.
• Initiate communication with the attending physician and interdisciplinary team for coordinating optimal care.
• Provide regular visits to reassess the patient’s needs and update the plan of care.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records, charts, progress notes, and timely care plans.
• Supervise and document delivery of care by the Home Health Aide as required by guidelines.
