Job summary

Frontier Hospice is seeking a compassionate Registered Nurse Case Manager (RNCM) for full-time work in Norman, OK. You will provide hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey.

Qualifications

• Current unencumbered RN license.
• At least 1 year RN experience in medical-surgical/acute care; hospice experience is a plus.
• Computer proficiency including EMR competency.
• CPR certification if required by state.
• Valid driver's license, vehicle insurance, and reliable transportation.

Responsibilities

• Provide hands-on hospice nursing care.
• Coordinate services and care plans with interdisciplinary team.
• Guide patients and families through the hospice journey.
• Deliver compassionate nursing care rooted in clinical excellence.
• Educate patients and families to participate in personalized care plans.
• Support on-call duties as part of the team.
