Job summary

Amedisys, part of the Optum family, is hiring an RN Case Manager to coordinate compassionate hospice care in the Greenville, North Carolina area. You will assess patients, partner with the interdisciplinary team, and develop plans that meet each patient’s needs.

Responsibilities include patient assessments, documentation, family education, on-call duties, and collaboration with the care team. A comprehensive benefits package and growth opportunities are available.

Qualifications

• Current RN license for the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license with reliable transportation and liability insurance.
• 1+ years of nursing experience.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to hospice care disciplines as needed or document rationale for not referring.
• Maintain individualized patient care progress records with accuracy and timeliness.
• Record pain/symptom management changes/outcomes.
• Educate patients, families, and caregivers on care techniques.
• Communicate with clinical manager and care team.
• On-call responsibilities and on-call services to patients/families.
