Job summary

Amedisys, part of the Optum family, is hiring an RN Case Manager to coordinate compassionate hospice care, assess patients, and partner with the interdisciplinary team to develop plans for each patient’s needs.

This full-time role serves the Blackstone South area, with a Monday–Friday schedule and salary range. You’ll provide patient-centered home care, teach families, and collaborate for optimal outcomes.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• Valid CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Maintains individualized patient care progress records with accuracy and timeliness.
