Job summary

AccentCare, Inc. is seeking a Hospice Registered Nurse (RN) in San Antonio, TX. You will work with an interdisciplinary team to provide compassionate hospice care, supporting patients and families at life’s most delicate moments.

Qualified candidates hold an active Texas RN license with at least one year of nursing experience. The role offers full-time hours with autonomy to practice at the top of your license, backed by a mission-driven organization that prioritizes well-being.

Qualifications

• Active RN license in the state of practice (Texas).
• Minimum of 1 year of nursing experience preferred, hospice or home health a plus.
• Reliable transportation and valid driver’s license.

Responsibilities

• Provide individualized nursing care to maximize comfort and quality of life.
• Partner with patients, families, physicians, and the care team to create and carry out care plans.
• Perform comprehensive assessments to guide care decisions across physical, emotional, social, and spiritual domains.
• Educate and support patients and caregivers, meeting them where they are.
• Coordinate care across disciplines to ensure continuity and dignity; document progress.
