Job summary

Adoration Health is seeking a dedicated Hospice Registered Nurse Case Manager to join our team. You will deliver patient-centered, compassionate care, create individualized hospice plans, and collaborate with an interdisciplinary team in a supported environment.

Responsibilities include easing patient and family stress through education, precise documentation, and on-call coverage as needed. A nursing license in the state and CPR certification are required, with hospice experience preferred.

Qualifications

• Graduate of an accredited nursing program with current RN licensure in the operating state.
• One year nursing experience required; 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of hospice philosophy of care and clinical documentation.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient’s family and caregivers.
• Collaborate with interdisciplinary team and maintain timely documentation.
• Participate in on-call rotation as required by the local branch.
