Above & Beyond Home Health Care and Hospice Agency in Monticello, IA is seeking a dedicated Home Health Hospice Case Manager. The RN will provide professional nursing care to hospice clients as prescribed by the physician, overseeing assessment, care planning, interventions, and overall outcomes.

This role requires state licensure and a focus on patient-centered, ethical care. The position emphasizes developing individualized care plans, coordinating with the IDT/IDG, and guiding families

Qualifications

• Graduate of an approved Nursing education program.
• Current Registered Nurse licensure in the state of Iowa.
• Must have one year of professional nursing experience; home hospice experience preferred.

Responsibilities

• Maintains current knowledge of applicable laws and agency policies to ensure ethical and professional care.
• Develops and implements an individualized Plan of Care focused on outcomes to improve quality of life for patients.
• Coordinates care with the IDT/IDG and other disciplines; makes referrals for evaluation and care as needed.
• Informs clients/caregivers about services, responsibilities, and payment obligations prior to initiating care.
• Applies nursing judgment and evaluation to formulate interventions as prescribed by the physician.
