Beacon Hospice, Amedisys, part of UnitedHealth Group, is seeking an RN Case Manager to coordinate compassionate hospice care by assessing patients, partnering with the interdisciplinary care team, and developing plans that support each patient’s needs.

You’ll perform patient assessments, document progress, teach families, and participate in on-call services. The salary range is $79,600 to $119,400 annually, with a comprehensive benefits package and the supports of a national health system.

Formación

• Current RN license for the assigned state(s).
• 1+ year of nursing experience.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsabilidades

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
