Job summary
James River Home Health and Hospice in Harrisonburg, VA seeks an experienced RN Case Manager for Hospice to support patients at home. You will manage a home caseload, collaborate with the interdisciplinary team, and ensure a seamless, high-quality care journey across the service areas of Harrisonburg, Staunton, Waynesboro, Broadway and Charlottesville.

Qualifications
- Must be a Registered Nurse with a valid Virginia license
- At least one year of home health experience preferred
- Graduate of an accredited nursing school
- Must have a valid driver’s license and a reliable car
- OASIS and HCHB experience PLUS!
Responsibilities
- Provides skilled nursing care to homebound patients per the Plan of Care
- Coordinates multidisciplinary services including admissions, transfers and discharges
- Educates patients and families on health maintenance and disease prevention
- Completes timely electronic documentation in Homecare Homebase and offers emotional support
