Qualifications

• Must be a Registered Nurse with Virginia license.
• Home health experience preferred (1 year).
• Graduate of an accredited nursing school.
• Valid driver’s license and reliable car.
• OASIS and HCHB experience preferred.

Responsibilities

• Administer nursing care to homebound patients.
• Establish treatment plans per Plan of Care.
• Monitor and report changes in patient symptoms.
• Coordinate with Interdisciplinary team for admissions/transfers/discharges.
• Educate patients and families on health maintenance.
• Document visits in Homecare Homebase.
• Provide emotional support to patients and families.
