The RN will manage a caseload, conduct assessments, develop care plans, and collaborate with an interdisciplinary team. The position offers a mix of facility and home-based hospice care, with on-call duties and a 40-hour work week.

Qualifikationen

• 1–2 years of recent acute care experience in an institutional setting is required.
• 1–2 years of recent home care experience is preferred.
• Graduation from an accredited school of nursing is required.

Aufgaben

• Complete initial and ongoing patient/family assessments to determine hospice needs.
• Develop and revise a care plan based on nursing diagnosis and patient goals.
• Administer medications and treatments as prescribed.
• Counsel patient and family on nursing care and self-care techniques.
• Coordinate patient care across the hospice team and community resources.
• Participate in on-call duties as defined by policy.
