Qualifications

• Current RN license, state where you work
• Current CPR certification
• Valid driver's license, reliable transportation and liability insurance
• Preferred: 1+ years of nursing experience

Responsibilities

• Performs initial nursing assessment on new admissions
• Coordinates and revises the plan of care with staff
• Communicates patient progress to the clinical team
• Maintains individualized patient care records per policy
• Records pain and symptom changes and outcomes
• Educates families on condition and care needs
• Makes referrals to hospice disciplines as needed
• Performs on-call duties for patients and families
• Performs other duties as assigned
