Care Initiatives Hospice is seeking an RN Case Manager to provide compassionate, patient-centered care across home, long-term care, assisted living, or hospital settings. You will meet patients where they are and coordinate with the care team to implement tailored plans of care.

Ideal candidates will hold an Iowa RN license, CPR certification, hospice experience, and be willing to travel within the region. This role emphasizes education for families and ongoing assessment to ensure quality

Qualifications

• Active Iowa RN license.
• Active CPR certification.
• Hospice or palliative care experience.
• Two (2) years of verifiable nursing experience preferred.
• Current driver’s license and ability to travel between locations.

Responsibilities

• Deliver high-quality patient care in the patient’s residence.
• Develop and oversee plan of care with physician oversight.
• Perform initial and comprehensive patient assessments.
• Conduct continuous visits and interdisciplinary assessments.
• Provide clear, concise clinical documentation.
• Counsel patients, caregivers, and families per plan of care.
• Update plan of care to meet patient-specific needs.
