Job summary

Care Initiatives Hospice in Cedar Falls, IA is seeking an RN Case Manager to provide comprehensive hospice care in patients’ homes or other care settings. You will work with physicians and a multidisciplinary team to coordinate care that respects patients’ wishes and enhances quality of life.

The role requires an active RN license, CPR certification, and hospice experience or willingness to obtain CHPN certification. Travel across multiple locations within your region is expected.

Qualifications

• RN license required.
• CPR certification required.
• Hospice or palliative care experience preferred.
• Willingness to travel between multiple locations within region.

Responsibilities

• Use clinical nursing knowledge, physical assessment, and procedural skills to deliver high quality patient care in the patient’s place of residence.
• Develop and oversee plan of care under the direction of the physician and ensure interdisciplinary involvement to achieve best outcome.
• Perform initial and comprehensive assessments on all patients per policies and procedures.
• Conduct continuous visits and interdisciplinary assessments of patients.
• Deliver quality care and complete clear, concise, and accurate clinical documentation.
• Provide patient, caregiver and family counseling and education as indicated in the plan of care.
• Develop and continually update plan of care that identifies needed services and care.
