Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to deliver compassionate, in-home nursing care within a structured hospice program. You will assess patients, implement and update plans of care, and coordinate with an interdisciplinary team to ensure high-quality, patient-centered care.

The role emphasizes education for patients and families, symptom management, and timely documentation, with on-call responsibilities as needed to support continuity of care.

Qualifications

• RN license required in state of employment
• Minimum 1 year RN experience in hospice, palliative care, home health, or related fields
• CPR certification required
• Current automobile insurance and valid driver’s license

Responsibilities

• Perform comprehensive patient assessments and reassessments (in-person/telephonic) and educate patients, families, and caregivers.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care including visit frequency.
• Coordinate care across interdisciplinary teams with physicians and other professionals.
• Educate patients and caregivers on disease process, self-care, medications, and end-of-life care.
