Job summary

Affinity Hospice is seeking a registered nurse/case manager to plan, organize and direct hospice care, drawing on nursing experience and community resources to support patients and families in home settings.

The role emphasizes patient care planning, interdisciplinary collaboration, and adherence to infection control and documentation standards in a home health environment. Requires RN license and CPR, with hospice experience preferred.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse.
• Current CPR certification.
• Minimum of two years of experience as a Registered Nurse in a clinical care, home health or hospice setting preferred.
• Must be a licensed driver with an automobile in good working order.
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills.
• Able to lift, position and/or transfer patients and lift supplies and equipment.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement/develop/document the patient plan of care.
• Provide care utilizing infection control measures that protect staff and patient (OSHA).
• Ensure continuity of quality patient care with proper documentation.
• Monitor assigned cases for third-party payer requirements.
• Demonstrate commitment to professional growth and competency.
• Promote agency philosophy and administrative policies.
• Perform on-call responsibilities for patients/families as assigned.
• Supervise hospice aides and licensed practical nurses.
• Perform other nursing duties as assigned to support department goals.
