Holisticare Hospice is seeking a Full Time Hospice RN Case Manager to join our compassionate nursing team delivering expert end-of-life care across Middlesex County with some coverage in Monmouth County. This role emphasizes hands-on nursing, care coordination, and family guidance within an interdisciplinary team.

Requirements include an active RN license, at least 1 year of clinical experience, EMR proficiency, CPR certification, and a valid driver's license. A $5,000 sign-on bonus is offered.

• Minimum one year experience as an RN in a medical-surgical/acute care setting; hospice experience a plus.
• Must be computer proficient in typing and various programs, including EMR background.
• Possess and maintains current CPR certification if required by state.

Schedule: Monday - Friday 8 am - 4:30 Ppm

Territory: Middlesex Co with some coverage in Monmouth Co

$94,000 - $99,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
