Job summary

Optum is seeking a Hospice RN Case Manager to provide compassionate, individualized hospice care across Brooke, Ohio, Marshall and Wetzel counties. The role requires coordinating with an interdisciplinary team, conducting patient assessments, and developing care plans to meet patients’ needs.

Qualifications

• RN license current and valid for the assigned state(s).
• At least 1 year of nursing experience.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Assess patients and collaborate with the care team to develop and implement a plan of care.
• Refer to other hospice care disciplines as needed or document rationale for not doing so.
• Maintain individualized patient care progress records accurately and timely.
• Record pain/symptom management changes/outcomes as appropriate.
• Educate patients, families, and caregivers on patient care techniques.
• Regularly communicate with the clinical manager and care team.
• Perform on-call responsibilities and on-call services as assigned.
• Other duties as assigned.
