Pikes Peak Hospice & Palliative Care in Colorado Springs is seeking an RN Case Manager to join a compassionate, collaborative team committed to clinical excellence and patient-family centered care. This full-time role offers a Monday–Friday schedule, competitive pay and a robust benefits package.

You will coordinate care with the IDT, develop care plans, provide skilled nursing and education, and support patients at home while traveling daily across Colorado Springs.

Qualifications

• RN license required.
• 1–2 years hospice/palliative/home health experience preferred.
• Current CPR certification required.
• Valid Colorado driver’s license and auto insurance.

Responsibilities

• Oversees and coordinates patient and family care per hospice philosophy.
• Develops and oversees care plans with patient, family, physician and IDT/IDG.
• Coordinates patient care through ongoing communication with physician and IDT/IDG.
• Ensures educational needs of patient and family are met to promote optimal care.
• Performs continuous assessment of physical, emotional, and spiritual needs.
• Participates in community education, staff training, and committees.
