Job summary

LUMINOUS Hospice is seeking an experienced RN Case Manager to lead a care team in delivering compassionate end-of-life care in patients' homes. You will collaborate with the Clinical Director, LPNs, LVNs, Aides, MSW and Chaplain to ensure coordinated, high-quality service.

You will oversee home care plans, educate families, perform assessments, and ensure timely documentation while maintaining compliance with state and federal regulations.

Qualifications

• Current State license as a Registered Nurse.
• Current driver's license.
• Current CPR Certification.

Responsibilities

• Coordinates and oversees all aspects of patients' home care.
• Collaborates with patient care teams to develop and implement care plans.
• Educates families on patient condition, needs and care.
• Performs head to toe physical assessments.
• Checks vital signs and manages medications.
• Administers and maintains catheters.
• Complete documentation timely and accurately.
• Supervises HHAs.
• Regularly communicates patient progress to the clinical manager, physician, and care team.
• Other duties as assigned.

Requirements

Licensure

Current State license as a Registered Nurse.

Experience

Two (2) years' experience as a Registered Nurse in a clinical care setting and one (1) year full-time hospice preferred.
