Empath Health’s Tidewell Hospice, part of the Empath Health family in Florida, seeks an RN Case Manager to join the South Facilities Team. Schedule is Monday–Friday, 8a–5p, serving from Venice to Punta Gorda.

The RN Case Manager develops care plans, coordinates with families, and delivers compassionate hospice nursing care. Requirements include a Florida RN license and a year of acute-care experience; hospice experience is preferred.

Formación

• Graduate of approved nursing program.
• Florida RN license is required.
• Minimum one year experience in an acute-care setting providing direct patient care.
• Hospice nursing experience preferred.

Responsabilidades

• The RN Case Manager begins at admission, creates and maintains a plan of care, and coordinates for patient and family interaction throughout the hospice experience.
• Reviews and updates the care plan, physician orders, and pharmacy profile; educates patients and families about disease processes and available interventions.
