• Graduate of an accredited nursing program.

Responsibilities

• Complete comprehensive patient and family assessments to determine hospice needs.
• Provide nursing care using the nursing process and plan of care.
• Develop and revise plans of care with the interdisciplinary team.
• Administer medications and treatments as prescribed.
• Counsel patients and families on nursing and related needs.
• Document clinical notes and report changes to physicians.
• Participate in on‑call duties and coordinate resources.
